This notice describes how medical information about you may be used and disclosed and how you can get access to this information. Please review carefully.

All retail pharmacies are now required by law to inform their patients of their policy regarding their protected health information. McElroy Pharmacy has been and continues to be bound by professional standards of confidentiality that are even more stringent than those required by law. Therefore, we have always protected your right to privacy.

McElroy Pharmacy keeps a Patient Profile for you to enable you and your doctor to know about your treatment (specific drugs used for each disease), payment (enables year-end medical expenses to be totalled) and health care operations (identification, assuring the correct medication is given to the right patient) We will always share relevant information with you or your doctor when requested. Any other disclosure of information requires your written authorization, which may be revoked at any time.

You have the right to request restrictions on uses and disclosures of your protected health information, even if we do not agree with your request. You have the right to receive confidential communications of health information; the right to inspect and copy protected health information; the right to amend protected health information; the right to receive an accounting of disclosures; and the right to obtain a paper copy of this notice upon request.

Every effort will be made to keep your personal health information confidential. In the event that your privacy rights have been violated, please address any and all complaints to Jeff Eschelman, RPh by calling 626-2222. If the matter is unresolved, you have the right to file a similar complaint with the Department of Health and Human Services. There will be no retaliation against any individual filing a complaint.

McElroy Pharmacy reserves the right to update these policies but will notify you upon modification.

We retain records relating to professional services that we provide so that we are better able to assist you with your healthcare needs. In order to guard your personal health information we maintain physical, electronic and procedural safeguards that comply with our professional standards.
